EQUIPMENT INSURANCE

Please fax proof of insurance to: Insurance Service Center 1-305-506-0094. The certificate should include the customer’s business name, location address of the equipment, a detailed equipment description, dates of coverage, and the contract number. Financial Pacific Leasing must be listed as Loss Payee. Please call 1-888-423-6722 with questions.
